Biography

Amy O’Linn DO SFHM FACP has served patients as a Cleveland Clinic hospitalist physician for 14 years in the Cleveland Clinic system, and has had numerous leadership roles, including President of the Lake Erie Chapter of the Society of Hospital Medicine (2011-2014); Site director for hospital medicine at Fairview hospital (2013-2018); and Regional Hospital Medicine Quality Improvement Officer (2018-Present).As physician lead for Enterprise Readmission Reduction since 2018, Dr. O’Linn has led numerous efforts to assure patients a durable discharge, including operationalizing the Discharge Medication Reconciliation hard stop for roughly 170,000 patients across the Enterprise annually (2018/2019); improving the communication from hospital to skilled nursing facility with the DrConnect enhancements (2019); expanding the Individualized Care Plan work from a local effort to Enterprise-wise in 2020; and finally leading the Readmission Reduction Team of Teams in 2021. Dr. O’Linn is passionate about maintaining the safety and quality of the hospital experience for our patients and giving patients the best chance at a durable hospital discharge. In addition to her Quality work, Dr. O’Linn serves her peers as an Advanced Peer Coach and has been trained in the Cleveland Clinic’s Emerge Stronger program.

Dr. O’Linn currently serves as co-chair for National Quality Forum Admissions and Readmissions committee, which influences decision-making on a national scale.
