Biography
Dr. Hayes is board certified in Family Medicine by the American Board of Family Medicine and is an active member of the American Academy of Family Physicians. Dr. Hayes earned her medical degree from Lake Erie College of Osteopathic Medicine in 2013 and completed her Family Medicine residency at Washington Health System in Washington, Pennsylvania in 2016.
Dr. Hayes is dedicated to empowering patients to become advocates for their health. She takes pride in collaborating with her patients to achieve wellness, encouraging them to take an active role in their healthcare decisions. Her clinical expertise encompasses the management of chronic conditions such as hypertension, hypercholesterolemia, and diabetes, as well as acute medical issues. Dr. Hayes provides comprehensive care to patients of all ages, from newborns to adults to geriatrics, with a particular passion for serving rural populations and enhancing their access to healthcare.
In addition to her clinical responsibilities, Dr. Hayes is serving as a member of the Primary Care Institute’s (PCI) senior leadership team as the Vice Chief of Clinical Operations for the East Submarket, effective 2024. Her focus in this role is driving and managing PCI practice day-to-day performance in various critical areas such as access, quality, safety, and patient experience. She works collaboratively with section heads, ambulatory operations, and APP leaders to promote primary care practice teamwork, stability, and staffing predictability. In her role, Dr. Hayes also collaborates with different Institutes and service lines within Cleveland Clinic Foundation to ensure seamless communication and coordination of care with primary care practices.
Dr. Hayes additionally has been appointed as the Medical Director for Community Health Equity for the East Submarket, effective April 2025. She reports to the Chief of Staff Office and working in partnership with the Director of Community Health Equity to oversee and drive the community health strategy and its implementation. One of her primary focuses will be aligning strategies with the community health needs assessment for the East Submarket, with a particular emphasis on Lake County as a priority region. Her role encompasses finalizing the community health strategy and creating programs based strategic data in the region. She is working closely with external organizations to build strong partnerships to create programs to address health needs of residents of Lake County and surrounding geographies with the goal to improve access and delivery of care to patients.

I think Dr. Hayes is great because she listens without judgement. She is very knowledgeable but also works with me if I have concerns about prior sensitivities to a drug for example among other things. She trusts my life long experience of living in a body that has autoimmune issues and I trust her.
